Name of the Patient : Abc XyzDaruwlmn / M / 28 yrs.
Referred by : Dr. Abc Xyzdar.
Examination : M.R.I. of the Lumbo-sacral Spine.
CLINICAL PROFILE :
C/O backache radiating to BLE.
EXAMINATION :
M.R.I of the lumbo-sacral spine was performed using the following parameters :
5 mm thick T1 Weighted and T2 Weighted sagittal images.
5 mm thick T1 Weighted and T2 Weighted axial images.
OBSERVATION :
The L4-L5 and the D12-L1 intervertebral discs show loss of water content with reduction in height of the D11-D12 and D12-L1 intervertebral discs.
There is sacralization of the L5 vertebral body which is as marked on the film.
There is a left paracentral disc extrusion with posterior peridiscal osteophytes at the L4-L5 level with antero-lateral indentation of the thecal sac. The extruded disc portion is seen to migrate inferiorly indenting the left L5 nerve root.
The L4-L5 facet joint on the right show slight degenerative change.
Small posterior disc bulges are identified at the L3-L4 and D12-L1 levels with posterior peridiscal osteophytes at the D12-L1 level.
The lumbar vertebral bodies and the remaining intervertebral discs reveal normal signal intensity. The rest of the facet joints and the visualized pre and paravertebral soft tissues are unremarkable.

The conus medullaris terminates at the D11-D12 level and the thecal sac terminates at the S1 level.
The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :
21.0 mm at L1-L2
20.0 mm at L2-L3
20.0 mm at L3-L4
13.0 mm at L4-L5
13.0 mm at L5-S1.
The cervico-dorsal spine was screened with 4 mm thick T2 Weighted sagittal images and shows small posterior disc bulges in the cervical region.
IMPRESSION :
1. Sacralization of the L5 vertebral body.
2. A left paracentral disc extrusion with posterior peridiscal osteophytes at the L4-L5 level with an extruded disc portion migrating inferiorly and indenting the left L5 nerve root.
