Examination : M.R.I. of the Right Hand.
CLINICAL PROFILE :
H/O spraining the right hand 4 months back with pain and swelling since then.
EXAMINATION :
M.R.I of the right hand was performed using the following parameters :
5 mm thick T1 Weighted and STIR axial images.
5 mm thick T1 Weighted and T2 Weighted (with fat saturation) sagittal images.
5 mm thick T1 Weighted and GRASS (with fat saturation) coronal images.
OBSERVATION :
There is evidence of a large mass lesion arising from the first metacarpal of the right hand and measuring approximately 8.0 x 6.0 x 6.0 cms. This lesion shows mixed signal characteristics. Few areas are mildly hyperintense to normal muscle on the T1 Weighted images and turn markedly hyperintense on the T2 Weighted, STIR and GRASS images. Some areas are markedly hypointense on the T1 Weighted images and turn hyperintense on the T2 Weighted, STIR and GRASS images and these may represent cystic/necrotic changes. Some areas are hyperintense on all the pulse sequences and may represent areas of haemorrhage/proteinaceous content.
This lesion cannot be separated from the adjacent thenar and pollicis muscles. There is probable involvement of the proximal first metacarpo-carpal joint with erosion of the trapezium. There is scalloping of the lateral aspect of the shaft of the second metacarpal.

The rest of the visualized metacarpal and phalanges are unremarkable.
IMPRESSION :
In a known C/O giant cell tumor, the MRI features are suggestive of a large mass lesion arising from the first metacarpal of the right hand and measuring approximately 8.0 x 6.0 x 6.0 cms. as described.
